Job Description

Carers Victoria is the peak body representing the needs and interests of unpaid carers across Victoria. We’re a for-purpose organisation working to ensure unpaid carers are recognised, supported and connected. We provide advice, respite, education and advocacy. 

About the role

The Practice Leader - Service Delivery & Innovation will provide operational leadership with a strong focus on our aged care services, particularly the Commonwealth Home Support Program (CHSP) Flexible Respite program.

This role will lead a small team in delivering high-quality, innovative services that align with the evolving Aged Care Act reforms. You will champion continuous improvement, professional development, and collaborative partnerships to ensure our services remain effective, sustainable and responsive to the needs of carers.

While the focus is on aged care, this role is designed with future growth in mind and may expand to encompass additional carer related services.

What you’ll be doing

  • Oversee and coach a small team delivering flexible respite services, including support for high-risk and complex cases
  • Drive continuous professional development aligned with sector best practices and aged care reforms
  • Design and deliver innovative models that improve access to respite services and enhance outcomes for carers
  • Embed the Aged Care Act principles of choice, control and participation into service delivery.
  • Ensure compliance with all funding, contractual & legislative requirements
  • Oversee brokerage services and ensure efficient use of program funding in line with guidelines
  • Monitor service effectiveness and drive continuous improvement using the Consumer Outcomes Framework and Aged Care Quality Standards
  • Develop and implement action plans to expand carer access to CHSP/Flexible Respite services
  • This role will also be hands on, with the Practice Leader directly engaging with carers through service planning and individualised support.

 

What we’re looking for

You are forward thinking with strong knowledge of aged care services and reform, especially within he CHSP/Flexible Respite space. You bring a deep commitment to improving carer outcomes, along with a hands on approach to service innovation, compliance and team leadership.

You will bring:

  • Proven experience in aged care or carer support services
  • Strong understanding of the Aged Care Act reforms and CHSP program
  • A track record of delivering innovative service models
  • Excellent operational, project management, and reporting skills
  • Ability to manage complexity and drive outcomes in a changing policy environment
  • Highly developed interpersonal, communication and stakeholder engagement skills.
